Referrals & Enquiries
Thank you for your interest in our services. Our clinic is focused on providing safe, high-quality clinical care to your patients and to facilitate an efficient enquiries/referral system.
How to make a referral
STEP 1: Submit a referral.
This can be done via email or Medical Objects or via our website through the following submission forms:
- To submit a General Referral, pleaseclick here
- To submit a Skin Lesion Referral Form, please click here
- For general advice about a patient or enquiries about our services, pleaseclick here
Please note that these are referral forms only for doctors to complete and will only be accepted with a valid provider number. If the form is submitted without a valid provider number, it will NOT be processed and the patient will NOT be able to make an appointment.
If you are a health practitioner who doesn't have a login, you can get your password via this application here.
STEP 2: Ask the patient to call our office for an appointment.
Please ask your patient to ring our office for an appointment (07) 3353 6165 during office hours (Mon-Fri 9am to 4pm, excluding public holidays). Please note that no appointment can be made until there is a valid referral in our system.
Due to increasing administrative burden, numerous unanswered phone calls, and associated higher percentage of no-shows, we no longer initiate contact with patient to make appointments. Patients will need to ring our office to make an appointment.
If you would like an urgent appointment for your patient, please do not hesitate to call and speak to one of our helpful staff members on 3353 6165 (Mon-Fri 9am-4pm, excluding public holidays).
STEP 3: We will keep you informed.
If we have received a referral from you and your patient has not contacted us for an appointment, you will receive notification from us. This notification will be issued at 2 weeks after referral received for all skin cancers (or suspected skin cancers), and for non-urgent elective surgeries, 1-2 months after referral received.
